Start/Stop Service

There are three online forms for utilities.

1. Start Service
   This form is for individuals wishing to set up utility service with the City of Plymouth.
    
2. Discontinue Service
   This form is for current utility customers who wish to discontinue service.
    
3. General Inquiry
   This form will help us respond to your request regarding general utility inquiries.
